Please note that applicants are required to submit a current CV along with a letter of support provided from the department chair (Pediatrics or Internal Medicine) confirming institutional backing of an additional fellowship trainee. The letter should also include the fellow’s opportunities for research, patient care, and leadership development.

Question 8 Attest that the fellowship program, as named above, is fully accredited by the Accreditation Council for Graduate Medical Education (ACGME) and considered in good standing (not on probation).
Question 9 Have you (the Training Program Director) been an active member of the American College of Allergy, Asthma and Immunology (ACAAI) for two years prior to submission of this application?
If yes, please provide the number of years you’ve been a College member along with a comprehensive overview of your ACAAI involvement, including specific roles, responsibilities, events participated in, and any notable contributions or achievements over the past five years. If no, please explain why not.
Question 10 Amount Requested. Specify how much you are requesting. Please note that the amount per year may not exceed $125,000.
Question 11 Are you submitting an application to fill a current ACGME-approved position, or are you seeking an increase in complement?
Question 12 If you are seeking an increase in complement, have you submitted an application to ACGME?
Question 13 Describe the need for an additional funded fellowship position and how the institution will support the fellow-in-training.
Question 14 List the key faculty at your institution and indicate who are ACAAI members. Describe each faculty member’s involvement in the College.
Question 15 The program must demonstrate that its previous fellows-in-training have regularly attended the ACAAI Annual Scientific Meeting. Provide a list of current and former fellows-in-training and outline their involvement at the ACAAI Annual Scientific Meeting over the last 5 years.
Question 16 List how many allergy fellow(s)-in-training the program currently trains and indicate their board certification (pediatrics or internal medicine).
Question 17 Outline the program’s educational leave policy including opportunities for fellows-in-training to attend national educational meetings including the ACAAI Annual Scientific Meeting.
Question 18 As it’s the goal of the College to increase the number of fellowship trained allergists practicing in the U.S., please outline what steps you plan to take when selecting a candidate to help ensure they will remain practicing in the U.S following completion of their training.
