CMS proposes updated RVUs for 2027

CMS estimates zero percent overall impact on allergy/immunology allowed charges for 2027 in the 2027 Proposed Medicare Physician Fee Schedule. However, CMS’ figures are averages for the specialty and do not reflect the impact on individual practices or services.

To estimate the impact of the proposed changes on reimbursement for you or your practice, we suggest plugging CMS’ new proposed RVUs into a spreadsheet with your historical distribution of services and the new proposed conversion factor.

Reimbursement for services under the Medicare Physician Fee Schedule reflects, in part, the relative resources typically required to furnish the service. CMS captures those resources by assigning relative value units (RVUs) to each service that account for three components:

  • Physician work
  • Practice expense associated with providing the service
  • Malpractice expense

In general, RVUs are then adjusted for geographic cost differences and multiplied by the applicable conversion factor to produce the payment rate.

As required by statute, CMS proposes to implement two conversion factors for 2027:

  • One for qualifying alternative payment model (APM) participants
  • One for practitioners who are not qualifying APM participants

The proposed 2027 qualifying APM conversion factor would be $33.1693, representing a 1.19% decrease from the 2026 conversion factor of $33.5675.

The proposed 2027 non-qualifying APM conversion factor would be $32.8409, representing a 1.68% decrease from the 2026 conversion factor of $33.4009.
