UnitedHealthcare requires prior authorizations for Nucala

Beginning Jan. 1, 2017, UnitedHealthcare will require prior authorizations for Nucala. This requirement applies to UnitedHealthcare national commercial plans, including affiliate plans such as UnitedHealthcare of River Valley, UnitedHealthcare Oxford, and Neighborhood Health Partnership.

Look up the prior authorization requirements for various UnitedHealthcare plans.

You can request a prior authorization in one of three ways:

  1. Visit the UnitedHealthcare website.  Under Notifications/Prior Authorizations at the top, select Notification/Prior Authorization Submission. Then sign in with your Optum ID. Or go to the sign in page directly.
  2. Call the Provider Services Number on the back of the patient’s insurance card.
  3. Complete a prior authorization form and fax to 866-756-9733. You can find the form in the UnitedHealthcare website Clinical/Pharmacy section.

UnitedHealthcare will inform you of coverage determination within 15 calendar days of receiving all required information.
