General requirements:

  • Report up to 6 quality measures, including one outcome measure or high-priority measure, for at least 90 days.
  • Must report on at least 50% of all patients, regardless of payer (exception: if reporting by claims must report on at least 50% of Medicare Part B patients)

Bonus options:

  • Two points for submitting an additional outcome measure
  • One point for submitting an additional high priority measure
  • One point for using CEHRT to submit all measures

Improvement Activities (IAs)

Improvement Activities (IAs) – New category

2017 category weight:

  • 15%

General requirements:

  • Attest that you completed up to 4 improvement activities for a minimum of 90 days.
  • If a small practice (< 15 providers) or rural or health professional shortage area, then attest that you completed up to 2 activities for a minimum of 90 days.
  • There are 92 possible improvement activities to choose from!

Advancing Care Information (ACI)

Advancing Care Information* (ACI) – Replaces Meaningful Use

2017 category weight:

  • 25% (optional for PAs / NPs)

General requirements:

Note: you can earn up to 155% maximum score including bonuses, but your score will be capped at 100%.

  • Fulfill all 4 required base measures for a minimum of 90 days (50% weight):
    • Security risk analysis
    • E-prescribing
    • Provide patient access
    • Health information exchange
  • Plus report up to 7 optional 2017 ACI transition measures (90% weight)

Bonus options:

You can report these if your technology is certified to either the 2014 or 2015 Edition or if you have a combination of 2014 and 2015 Editions. Most groups will fall under this category, although you also have the option of reporting the Advancing Care Information Measures & Objectives if your technology is certified to the 2015 Edition.
